I give permission for photographs and videos of my family members taken at Explorers functions to be uploaded to the closed (private) Explorers Facebook group. I agree not to share photographs and videos outside of the closed Facebook group that include members who are not part of my family.

I agree that my minor child(ren) has my consent to participate in Explorers’ events or activities. I further provide my consent for Explorers to seek emergency treatment for my minor child(ren) if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment. I assume any risk of harm or injury which might occur to the participant due to his/her participation in an Explorers event or activity. I release the Explorers Homeschool Co-op and its representatives from all liability, costs and damages which might arise from participation in any Explorers’ event or activity. By signing below I am indicating that I have read and am in full agreement with the bylaws, policies and guidelines of Explorers Homeschool Co-op.

1. You must be willing to commit to the Explorers Co-op for the entire 2024-2025 school year.
2. You must be willing to teach or assist in at least 2 class periods each time Explorers Co-op meets.
3. A non-refundable fee of $75.00 per family ($40 for first time spring registration) is required in order to reserve your spot for the 2024-2025 school year (this does NOT include your supplies for the classes or any applicable class fees).
4. By signing this form, you must have thoroughly read the co-op guidelines and agree to all that is stated in them. You also understand the commitment involved in joining the Explorers Co-op and you are willing to follow the guidelines to the best of your ability.
