Children are divided up into the following groups:

N (Nursery - Birth-2yo)

Pre-K (3-4yo)

K5-2nd grade (Usually ages 5-8)

3rd-5th grade (Usually ages 8-11)

6th-8th grade (Usually 11-14)

9th-12th grade (Usually 14-19)

Please list the grade level for your child that reflects what group you will be placing them into. (Ex. Your 11yo is doing 6th grade at home, but wants to be in the 3rd-5th grade class with a 5th grade friend. List that child as 5th grade.)

**Children are only allowed to move 1 grade up or down based on public school age requirements. (Ex. Based on your child's age, your child is considered a 5th grader. Your child can be moved up to 6th-8th grade but not down to K5-2nd grade. In Florida, a Kindergartener is 5 on or before 9/1. If your child turns 5 on 9/2 they are considered Pre-K but can be moved up to K5 classes.)

I agree that my minor child(ren) has my consent to participate in Explorers' event or activities. I further provide my consent for Explorers to seek emergency treatment for myself or my minor child(ren) if necessary.

I agree to accept financial responsibility for the costs related to this emergency treatment. I assume any risk of harm or injury which might occur to the participant (parent or child) due to his/her participation in an Explorers event or activity. I release the Explorers Homeschool Co-op and its representatives from all liability, costs, and damages which might arise from participation in any Explorers' event or activity.

You must be willing to commit to the Explorers Co-op for the entire 2025-2026 school year.
You must be willing to teach or assist in at least 2 class periods each time Explorers Co-op meets.
A non-refundable fee of $100 per family ($50 for first-time spring registration) is required in order to reserve your spot for the 2025/2026 school year. (This does NOT include your supplies for the classes or any applicable class fees).
By signing this form, you must have thoroughly read the co-op guidelines and agree to all that is stated in them. You also understand the commitment involved in joining the Explorers Co-op, and you are willing to follow the guidelines to the best of your ability.
